Partner Spotlight: Active Living By Design

Tell us a little bit about your organization.

Active Living By Design (ALBD) advances community-led action with evidence-informed, practice-based strategies to ensure health and well-being for all. We are strategic thoughtful partners for developing inclusive, cohesive, and resilient community health initiatives focused on equity. We believe in the power of people to shape their communities into healthier places to live. At local, regional, state, and national scales, we help communities turn effort into action by honoring and amplifying their unique contexts, cultures, and assets. Drawing on our experience working with hundreds of communities, we ensure that investments of time, energy, and funding lead to a sustained culture of health and well-being.

Explain your partnership with Care Share in 3-5 sentences.

We partner with Care Share to support the Community Centered Health (CCH) Initiative of Blue Cross Blue Shield of North Carolina Foundation. The initiative aims to advance prevention and improve population health in North Carolina through collaborative efforts by community members and the health care and community-based organizations that serve them. As an integrated team with Care Share, we’ve provided training, coaching, “action learning”, and a learning network for NC-based communities experimenting with CCH approaches. Our joint work emphasizes:  community capacity building with a strong emphasis on community engagement and resident leadership development, a shift in clinical practice to emphasize social determinants of health and integrate with community improvement efforts, and policy, environmental and systems approaches to sustainable community change.

 What inspires or motivates you/your organization to partner with Care Share?

We’ve learned a lot about the health care safety net in NC from Care Share, and Linda Kinney has been a skilled, flexible, reliable and fun partner throughout our almost three years working together.   Since our values overlap a lot and our skills are complementary, we’d love to find more health equity  and clinical-community coordination projects to work on together in the future.

The Affordable Care Act Continues to be the Law!

On March 24th 2017, the health care bill to repeal and replace the Affordable Care Act was withdrawn because there was not enough support for the bill to pass.    Speaker Paul Ryan said “Obamacare is the law of the land and this will be the case for the foreseeable future.”

It’s more important than ever for us to educate consumers about the ACA and enroll them into coverage both during the current Special Enrollment Period and next Open Enrollment Period in order to counter the message that the ACA is in a death spiral.  We want the ACA to succeed in order to ensure health insurance coverage for North Carolinians who need it the most.

Team Spotlight: Kellan Moore

As Executive Director, Kellan is responsible for the overall strategic direction, management, fundraising and evaluation of Care Share Health Alliance.  She spends more than half of her time working in partnership with local communities to improve access to care and health coverage for our uninsured neighbors.

What do you enjoy about your position with Care Share?

I really enjoy facilitating strategic planning processes, public speaking and sharing the great impact collaborative networks are having across the state. I’ve been fortunate to work with an amazing board and staff to create programs, policies and approaches that leverage resources, supports our collective learning, and fosters collaboration with the goal of achieving health equity for all.

What inspires you about your work with Care Share?

Our commitment to building on local assets and creating programs and approaches that improve health at the individual and community levels.  I really think people just want to be healthy so they can work and care for their families.  I believe our collective work helps people live more productive, healthy and happy lives.  In the process, I’m afforded the opportunity to meet amazing people from different sectors, build partnerships, and learn from their wisdom and expertise. I’m always inspired by the people I get to meet and their commitment to doing this important work as well as the volunteer spirit of the providers who donate their time and services to care for uninsured people living in their communities. I’ve also been inspired by the more 280 community based organizations who participate in the NC Get Covered Coalition and are working so hard to get and keep people covered through the ACA.

What do you like to do in your free time?

Stay connected to friends and family, travel, try new restaurants, wine tastings, hike, bike, read and watch movies. And, I love an afternoon nap on the weekends.

What have you been reading or watching lately?

Hillbilly Elegy, by JD Vance and The Gifts of Imperfection by Brene Brown.  I love to  watch Madam Secretary and all of the NCIS shows.

Have you been on any recent, meaningful trips?

I turned 50 last year, so I’ve had a wonderful year of travel from Napa Valley to Paris, France, and many places in between.  I love living in NC because I get to visit the mountains and the beaches, and all great places in between, while traveling to meet with communities.

Do you have a favorite quote or poem?

“If you want to go fast go alone, if you want to go far go together.”  ~African Proverb

Megan serves as the Program Coordinator for Care Share.  In her role, Megan provides facilitation and technical assistance to Collaborative Networks and Models across the state.  She collects and disseminates best practices to help improve processes and outcomes for Collaborative Networks and Models.  She leads communication efforts for Care Share.

What inspires you about your work with Care Share?

I love hearing the stories from Collaborative Networks about helping patients receive health care and services that have improved their health and their quality of life. I am inspired about moving our work with communities to address social determinants of health at a large scale. I like creating processes for people to come together and help them move toward a goal or outcome.

What do you like to do in your free time?

I love spending time outdoors- hiking, kayaking, swimming-  just being in the woods!  I enjoy spending time with my family, friends, dogs and ducks.  I love traveling from short weekend trips to extended international trips.   I am engaged in my community through active participation with my town’s parks and recreation advisory board and my church’s mission team.

Have you been on any recent, meaningful trips?

Yes!  I find meaning in every trip that I go on- it’s hard for me to choose just one!  Most recently I have traveled with my boyfriend to Colorado.  We hiked in The Rocky Mountain National Park, Gilpin Lake Trail, Fish Creek Falls- it was absolutely beautiful!   We visited with close friends, explored local towns, went to a concert at Red Rocks Amphitheater and most importantly shared a lot of laughs and happy moments.

What’s New?

Care Share has adopted a new vision and mission statement.   Our new vision is “Communities are healthy and committed to health equity for all.”  Our new mission is “Collaborating for health equity.”  We have also updated our guiding values which now includes looking at our work through a lens of intersectionality.

The new strategic plan has two new priorities areas. We will Engage in Social Determinants of Health (SDH) to Promote Health Equity. Care Share will primarily engage in SDH by serving as a neutral convener to help state and local partners align with groups who have a focus on addressing SDH needs at the community or systems level, and help Collaborative Networks and models connect into existing or emerging initiatives. We will also share best/[promising practices emerging from the field.

Care Share will Optimize Opportunities through Health Reform. Care Share will serve as a resource and neutral convener around the coordination and delivery of care in changing systems associated with health reforms, and the issues and opportunities impacting underserved people to improve their health. Care Share will continue to serve as the backbone organization/fiscal home for NC Get Covered Coalition of 280 organizations leading the implementation of outreach, education and enrollment functions.

Our Core Work Continues 

Care Share will continue to focus our work on Strengthening and Developing Collaborative Networks and Models. Care Share’s Technical Assistance (TA) team helps develop Collaborative Networks and Models that:

  • Provide access to and coordinate healthcare and other resources for underserved people
  • Implement promising practices for network development and operations
  • Participate in shared learning via peer-to-peer opportunities.
  • Leverage limited resources at state and local level

Care Share’s “strengthening” TA services for Collaborative Networks and models include the following:

  • Collaboration Building/Neutral Conveners to create solutions
  • Program design
  • Leadership coaching for directors/coordinators and community leaders
  • Facilitating peer-to-peer learning across networks, models and sectors
  • Webinars
  • Community Wide Planning and/or business plan development
  • Collaborative funding proposal development across networks and sectors

Care Share will also continue to Diversify funding for Care Share, Collaborative Networks and Models.  Care Share will diversify and expand funding by cultivating relationships with county, state, regional, and national funders, investors, partners and resources, and explore funding opportunities focused on the strategic plan and emerging opportunities that don’t have geographical restrictions. Care Share will continue to increase business opportunities for networks.

We are excited about our future work.  We envision new partnerships and collectively moving the work forward to help create healthy, equitable communities!

Congratulations to Madison Hardee for receiving the New Leaders in Advocacy Award!  This award is presented by the National Legal Aid and Defender Association.  It recognizes attorneys who exhibit extraordinary leadership early in their careers as civil legal aid or indigent defense advocates.  Madison is active on the Care Share Board of Directors and NC Get Covered Advisory Board.  Way to go Madison!

imac-mockupCare Share is excited to unveil our newly designed website! Visit the website at www.caresharehealth.org/. We wanted to make our new online home brighter, easier to navigate, more user-friendly, and mobile friendly by clearly describing the work of Care Share and who we are as an organization. Key features of the new design include:

  • An Interactive Map highlights each Collaborative Network and includes details like contact information.
  • A Searchable Resource section that includes best practices, shared forms, and webinar presentations.
  • A News section that will keep you posted on the latest hot topics. We can also share your important news too!
  • An Events Calendar where we will post information about upcoming meetings, events, webinars, conferences, and more. Check it often!
  • Plus much more!

We would like to thank Tomatillo Design, our web developer, for their work on our new site. Our team has worked hard to make the website a useful resource.
