A Collaborative Network is an entity comprised of multiple local partners who integrate medical, preventative, community, social, and economic resources to achieve collective outcomes through a coordinated system of care. Collaborative Networks have shared vision, purpose, priorities, strategies, and objectives aligned to improve the health people experiencing inequities.
Collaborative Networks typically offer services such as: eligibility screening and enrollment, connection to primary and specialty care, labs and diagnostics, medication and case management and navigation to community and social resources. This map represents 20 Collaborative Networks serving 36 counties. For more information on each Collaborative Network, visit the Collaborative Networks in NC page.
