Want to learn and share what’s happening at the state and local level to address this from multi-level approaches? Then join this webinar presented by the Division of Public Health in the North Carolina Department of Health and Human Services in partnership with Care Share Health Alliance and Community Food Strategies.

Participants will be able to:

Describe food security and healthier food access definitions and data sources
Identify the various roles of state and local level partners in addressing food security from a multi-level systems and asset-based approach
Locate at least 2 resources and strategies that can be helpful in this work
Share strategies working in their communities

This Webinar is an overview of the SFY 2020 Community Health Grant Request For Applications (RFA). Community Health Grants provide up to three years of funding to support safety-net organizations such as federally qualified health centers, state-designated rural health centers, free and charitable clinics, public health departments, school-based health centers and other non-profits that provide primary health care to underserved populations throughout North Carolina.

This Webinar is an overview of the SFY 2018 Community Health Grant Request For Applications (RFA). The Community Health Grant RFA (year one) is a competitive grant application. The Webinar will allow potential applicants an opportunity to review the RFA and seek clarification on any questions they may have as they complete the application.

Active Living By Design (ALBD)’s Community Action Model (CAM) is an evidence-informed framework for catalyzing healthier communities through comprehensive and integrated strategies. More than a dozen years of successful initiatives have deepened ALBD’s understanding of the community change process and have informed the CAM, which highlights the importance of a community’s context, defines six essential practices that undergird success, and outlines a three-phase action approach. It also presents expected impacts. This webinar will cover aspects of the CAM that can be useful to community coalitions and local leaders seeking a collaborative approach to creating healthier places and to funders seeking a tested approach for local investments.

This webinar discusses the diabetes burden in North Carolina and the collaborative work of the North Carolina Diabetes Advisory Council, NC Division of Public Health and stakeholders in developing and implementing North Carolina’s Guide to Diabetes Prevention & Management. The Guide is presented in detail including the NC diabetes facts, theoretical models and framework, and the role of the health care providers, employers, and communities in the implementation & evaluation process. The position paper (ADA-AADE- and American Academy of Nutrition & Dietetics) recommendations for DSMES are integrated emphasizing DSME benefits, support, reimbursement, and the algorithm with critical timing recommendations.
