Reducing the Burden of Diabetes North Carolinas Guide to Diabetes Prevention and Management

This webinar discusses the diabetes burden in North Carolina and the collaborative work of the North Carolina Diabetes Advisory Council, NC Division of Public Health and stakeholders in developing and implementing North Carolina’s Guide to Diabetes Prevention & Management. The Guide is presented in detail including the NC diabetes facts, theoretical models and framework, and the role of the health care providers, employers, and communities in the implementation & evaluation process. The position paper (ADA-AADE- and American Academy of Nutrition & Dietetics) recommendations for DSMES are integrated emphasizing DSME benefits, support, reimbursement, and the algorithm with critical timing recommendations.
